Endovascular Repair of Aortic Dissection Involving the Left Subclavian Artery by Castor Stent Graft: A Multicentre Prospective Trial

چکیده

The left subclavian artery (LSA) origin can be intentionally covered by stent grafts, to provide adequate proximal landing zones during thoracic endovascular repair for Stanford type B aortic dissections (TBADs). To preserve the LSA, a novel single branched graft, named “Castor” was designed and clinical trial conducted investigate its suitability. From April 2013 March 2015, 73 patients with TBAD were treated Castor grafts at 11 Chinese tertiary hospitals as part of arm prospective trial. There 50 acute (<2 weeks [68.5%]) 23 chronic (>2 [31.5%]). technical success rate 97% (n = 71/73). two failures caused occlusion branch section graft. four intra-operative endoleaks (two Ia, from LSA). endoleak 5% 4/73). one in hospital death no major complications. median follow up time 61 months (range 48–72 months). mortality 4/73) within year 7% six years 5/73). Two deaths unknown cause three not related aorta. new entry tears found on or distal edge graft retreated endovascularly. Six occlusions found, patency 93% 63/68). eventually disappeared according latest computed tomography angiograms. For TBADs needing anchoring may an easily manipulated, safe, effective treatment.

Endovascular Repair of Intra-thoracic Left Subclavian Artery Aneurysm with a Stent Graft

True aneurysms of the intra-thoracic portion of the subclavian artery are rare, comprising 3.5% of all peripheral aneurysms and the presence of associated aneurysms has been reported in as many as 46% of patients. The morbidity and mortality from standard surgical treatment is relatively high. Transluminal implantation of a covered stent graft has been reported as a feasible and safe procedure ...

Alternative management of the left subclavian artery in thoracic endovascular aortic repair for aortic dissection: a single-center experience

BACKGROUND Since the new 2009 guidelines for left subclavian artery (LSA) management using thoracic endovascular aortic repair (TEVAR), a few studies have been published about alternative LSA management. The objective of this study was to present the follow-up results of covered or revascularized LSA during TEVAR. METHODS From January 2010 to August 2012, 109 consecutive patients were treated...

Should intentional endovascular stent-graft coverage of the left subclavian artery be preceded by prophylactic revascularisation?

Thoracic endovascular aortic repair (TEVAR) has emerged as a promising therapeutic alternative to conventional open aortic replacement but it requires suitable proximal and distal landing zones for stent-graft anchoring. Many aortic pathologies affect in the immediate proximity of the left subclavian artery (LSA) limiting the proximal landing zone site without proximal vessel coverage. In patie...

Endovascular stent graft repair of aortic dissection type B extending to the aortic arch.

OBJECTIVE To analyse our experience with endovascular stent graft repair of descending aorta dissections that require supraaortic branch vessel revascularisation. DESIGN Retrospective study. METHODS From 2001 to 2009, 22 consecutive patients with the above dissections were retrospectively analysed. Inclusion requirement: aortic landing zone proximal to the left subclavian artery of less tha...

Endovascular repair of latrogenic subclavian artery perforations using the Hemobahn stent-graft.

PURPOSE To report the use of a new self-expanding endograft for percutaneous treatment of iatrogenic subclavian artery perforations. CASE REPORTS The subclavian artery of 2 patients was inadvertently cannulated during percutaneous attempts to implant a permanent pacemaker in one and catheterize the subclavian vein in the other. Because both patients had serious comorbidities, endovascular rep...
